How can I get a new covid-19 vaccination card?
With vaccine mandates becoming ever more prevalent, having proof of your vaccination is more important now than ever, and a replacement is possible.
If you lose your vaccine card, the first thing you should do is get in touch with the center that administered you the shots and make them aware of the situation. Pharmacies, medical centers, and hospitals should have a record of your immunization. However, if that is not the case, there are several things you can do to get a new card. With vaccine mandate deadline coming in just over a months time, it is more important than ever to have one.
Who to contact
According to CDC recommendations, you should contact your health department's immunization information system as soon as possible, they should have all your vaccination proof registered, or call (860) 509-7929 to ask for your official immunization record from the CT Immunization Information System. They will give you the steps to get a new card.
The CDC also has its own smartphone-based tool called "V-Safe" you can register on. It has step-by-step instructions on the CDC website. Some states, such as California and New York, have vaccine cards that can be stored on people's phones, making it impossible to lose it if you keep your phone on hand.

"This program is to track post-vaccine side effects and symptoms through a smartphone," assistant professor of emergency medicine Shideh Shafie said. "Register when you get your first vaccine, and the program will keep track of your vaccine information so that you can access it later, should you lose your card."
The CDC also recommends taking a picture of your card when you first receive it. This will make it easy to remember data on it for if you need to get a new one.
Why a vaccine card is important
While not all States have implemented them, vaccine mandates are becoming more and more common across the US. Federal workers will need to be fully vaccinated by December 8 or risk losing their jobs. Some mandates have come into force this week, causing some high profile job losses.
In one case, Washington State University fired its head football coach and four of his assistants on Monday for failing to comply with the state's vaccine requirement. The coach, Nick Rolovich, had applied for a religious exemption from the mandate earlier this month.
There are 12 states which have so far refused to enforce the federal vaccine mandate. These are: Arizona, Arkansas, Florida, Georgia, Indiana, Montana, New Hampshire, North Dakota, Oklahoma, Tennessee, Texas, and Utah.
