It’s official, if you’re on this list, you can claim a settlement payment of up to $25,000: the deadline is December 22, and all you need is a receipt
Time is running out to make a claim as part of a $30 million class action lawsuit settlement for a data breach at Integris Health.
Data breaches have become all too common as hackers use ever more sophisticated methods in their efforts to steal people’s personal information. Such was the case with Integris Health on 28 November 2023, when a threat actor managed to gain access to the files of nearly 2.4 million individuals.
The impacted information varied by person but the hacker potentially gained access to “name, date of birth, contact information, demographic information, and/or Social Security number,” states Integris Health.
While the Oklahoma-based company did not admit any wrongdoing, it has agreed to a $30 million class action lawsuit settlement. Individuals whose personal information was or may have been compromised in the data breach can receive up to $25,000 under the terms of the agreement.
How to claim Integris Health settlement payment
If you have received a notice of the settlement, you have been identified as a member of the class action against Integris Health. The health care company may have sent you a previous notice as well.
Class action members can claim up to $25,000 for documented losses resulting from the data breach. You will need to present receipts to prove those losses.
On the other hand, those without proof can claim up to $100, but depending on the number of successful claims, this pro rata cash payment amount could be less.
Integris Health is also providing three years of free credit monitoring services the three major credit bureaus. This will include up to $1 million of identify theft insurance.
Deadline to claim Integris Health settlement payment
Class action members must submit a valid claim by 22 December 2025. Links to the online form and a downloadable form are available on the Integris Health website. Remember that filing a fraudulent claim not only hurts other claimants but is also a crime.
Those who wished to opt out of the settlement had to have done so by 21 November 2025. If you choose to do nothing, you will not receive any settlement but will remain a member of the class action lawsuit and be bound by the releases.
A court hearing for final approval of the settlement will take place on 12 December 2025 at 11:00 am CT.
