- Proof of Identity: Driver's License, Passport, Country-issued ID, Green Card, etc.
- Proof of Boulder County Residency: Utility Bill dated and issued within the last 60 days, Pay Stub, Rental Agreement, etc.
- Proof of Income: 2023 Tax Form (personal or business), Pay Stub for the last 90 days from all wage earners in the household, etc.

Those selected to participate in the program will not need to take any additional action. They will begin receiving payments via their preferred payment method on the date indicated, either a direct cash transfer to a bank account or a debit card that will be reloaded monthly.
