Historic breakthrough in diabetes treatment: Japanese scientists take steps toward creating oral insulin

Diabetes is a chronic disease that affects more than 400 million people worldwide and makes it difficult for patients to regulate their blood sugar levels.

For type 1 diabetes, insulin is essential and is typically delivered through subcutaneous injections, although alternatives such as insulin pumps are also available. However, oral insulin may be closer to becoming a reality thanks to a recent breakthrough by scientists at Kumamoto University.

The advance centers on a solution that uses a cyclic peptide known as DNP peptide, which helps insulin cross the intestinal wall. To test this approach, the researchers used two different methods for oral drug delivery. The first, called the mixing method, combined a modified D-DNP-V peptide with zinc-stabilized insulin hexamers.

The second approach, known as the conjugation method, directly linked the DNP peptide to insulin. The team demonstrated that both treatments lowered blood sugar levels with similar effectiveness. According to Associate Professor Shingo Ito, the platform “offers a new route to deliver insulin orally.”

Drug bioavailability between 33% and 41%

Ito also noted that the technology “may be applicable to long-acting insulin formulations and other injectable biologics.” The platform achieved a drug bioavailability rate ranging from 33% to 41%, which is higher than results reported so far.

It is important to note that the initial trials were conducted in animal models, including both chemically induced diabetes models and animals genetically predisposed to diabetes. The treatment has not yet been tested in humans, so its true effectiveness and full potential remain unclear.
