49ers head coach Kyle Shanahan has quelled fears over Christian McCaffrey’s fitness to face the Titans this weekend, after San Francisco’s star running back was only a limited participant in practice on Thursday.
Why is McCaffrey on 49ers injury report?
The 49ers’ Week 15 injury report lists McCaffrey as suffering from an unnamed illness, but Shanahan told KNBR radio show The Sports Leader: “He’ll be alright.”
After playing just four times in an injury-plagued 2024, McCaffrey has started every game for San Francisco this season, as the 9-4 Californians chase a return to the postseason.
In his 13 starts in 2025, the 29-year-old has registered 85 receptions for 806 yards and five touchdowns, and 849 rushing yards for eight TDs, leading bookmakers to install him as the favorite for this season’s Comeback Player of the Year award.

In Week 15 of the 2025 NFL regular season, the 49ers host the Titans on Sunday, December 14, with kickoff at Levi’s Stadium scheduled for 4:25 p.m. ET/1:25 p.m. PT.

SF still in division title hunt
While the 2-11 Titans have already been eliminated from playoff contention, the 49ers return from their bye week sitting in a good position to bounce back from last year’s failure to make the postseason.
Currently on a three-game winning streak, Shanahan’s men occupy one of the NFC’s Wild Card berths - and remain in the race to win the NFC West division. Although San Francisco is presently third in the division, it is only one game behind the Los Angeles Rams and the Seattle Seahawks, who both hold a 10-3 record.
The NFL’s ‘playoff probability’ calculator gives the 49ers a 93% chance of making the postseason - a percentage that will rise to 95% if they beat the Titans on Sunday.
